PATH Outreach Services - Burrell Behavioral Health

Frontline services for the population with serious mental illnesses and those with co-occuring substance use disorders who are experiencing homelessness or at imminent risk of homelessness to find support with obtaining their basic unmet needs (i.e. IDS, Birth certificates, hygiene items/clothing voucher/seasonal items, screening for mental health services and, based upon engagement and enrollment in some instances, services for one-time assistance).

Case Management

PATH case management does not require insurance. The process begins with a screening for determination of eligibility. If the individual is in need of support to obtain medications, actively enrolled PATH clients will be evaluated for temporary medication payment assistance.
